Though you have failed to resolve the issue with 'Disk Utility', still you can try with the command-line utility, 'fsck', which scans the Mac volumes for any file system errors and fixes the same.

However, if you also fail with 'fsck', you have to make a fresh installation of the operating system using the 'Erase and Install' option. In this process, all you data from the Mac hard drive will get erased and you need to restore them from a valid backup, failing to which, you are left with the only reliable option, any Mac recovery software to recover back your formatted data.
