Bone-strengthening drug Fosamax became a blockbuster medicine in the United States and the rest of North America because of the millions of Americans dealing with osteoporosis. Although this bisphosphonate works in wonder against osteoporosis, it also carries undesirable effects, including a risk for osteonecrosis of the jaw. But what is osteonecrosis of the jaw and what must people in the world need to know about this severe dental problem?

In the recent past, the Food and Drug Administration (FDA) had discussed about health care providers not carefully prescribing biphosphonates to their patients. Consumer advocacy group Public Citizen named two groups who are not getting any benefits from Fosamax therapy. These populations who may be unnecessarily taking the drug are also putting themselves at risk of this jaw bone problem.

Osteonecrosis of the jaw, also known as ONJ and "dead jaw," is a condition in which an area of the jawbone is exposed. This bone tissue does not receive enough blood supply, which eventually leads to the its death or necrosis. The terms “Bis-Phossy Jaw” and BON are invented by scientists who refer to ONJ as having been brought by the use of bisphosphonates, such as Fosamax.

In the first stage of ONJ, patient would appear to have no symptoms until the disease develops, in which one could feel pain, swelling and redness usually associated with gum infection. ONJ is often diagnosed by the presence of exposed bone tissue, which more often lasts for more than eight weeks. {Furthermore, a dental procedure would often exacerbate or worsen the condition.|Moreover, dental surgeries like tooth extraction could aggravate the condition.

By and large, ONJ is managed with non-surgical interventions such as antimicrobial wash, antibiotic drugs, skin-applied antifungals and analgesics. When advanced stages are reached, surgical debridement or resection, together with nonsurgical interventions, might be performed. It if best for anyone who have the symptoms to inform his/her dental specialist know if he/she is on bisphosphonate therapy before any dental procedure – even if it’s just a minor surgery – is performed.

In July 2005, new revisions regarding ONJ risks by Merck on Fosamax label was made available on public as a reponse to the request from the FDA a few months before. Nonetheless, several long-term Fosamax patronizers claimed that Merck lacked proper disclosure of the said risk causing them to agonize such side effect and other backlash. This moved affected men and women to file Fosamax lawsuit against the pharmaceutical company.
